Diabetes Wellness Program

Diabetes is a life-changing, chronic condition. It requires an understanding of how foods, activity and medications affect blood sugar levels. To feel well and stay well, people with diabetes must learn to balance eating, exercise and diabetes medications so that blood sugar stays within a healthy range.

To assist those with diabetes in this complex balancing act, St. Agnes Hospital has developed several programs that focus on teaching the skills necessary to control blood sugar and to slow or prevent the complications of diabetes.

Individual Education

A certified diabetes educator is available by appointment, Monday through Friday, for one-on-one training in the use of diabetes medications (such as insulin), use of home glucose monitors, and management of high and low blood sugar.

Nutrition Consultation

A registered dietitian with specialized knowledge of the diabetic meal plans, offers individualized counseling by appointment on carbohydrate counting and weight management

Classes

Adult education classes are scheduled several times a year, both days and evenings. The friendly, small-group atmosphere offers participants the chance to discuss problems common to diabetes. Our program meets ADA guidelines to address the following topics:

  • Weight Management
  • Understanding diabetes
  • Exercise as therapy
  • Dealing with emergencies
  • Preventing complications
  • Glucose monitoring
  • Use of medication
  • Foot health
  • Stress and diabetes
  • Meal planning

(Pre-class evaluation is recommended so that an individual’s special needs can be identified and accommodated. A follow-up visit after the class is also available.)

Support Group

Financial

Services of the Diabetes Center are covered by most insurance plans, including Medicare. Financial assistance is available.
