- weigh at least 100 pounds over ideal body and/or
twice the ideal body weight with a BMI (Body Mass Index) greater

such as diabetes, hypertension, leg swelling, leg ulcers, reflux, stress
incontinence, menstrual irregularities, sleep apnea, obesity hyperventilation
syndrome, joint pain, heel pain, high cholesterol, pseudotumor
cerebri, shortness of breath, depression, etc.
- have exhausted all efforts to lose weight through
diet. Some insurance companies will require documentation
of these attempts.
