Preparing an Advance Directive
Advance directives are written instructions on your health care. These instructions include the name of someone who can make health care decisions for you if you are unable to do so. An explanation of these documents and blank forms can be found here. If you have an advance directive, please provide it to you doctor. If you do not have an advance directive prepared, you can prepare one and have it added to your medical record.
