Ensure Your Wishes Are Known

A durable power of attorney and a living will are known in health care as advance directives. They are documents in which you state what you want done in the event you cannot make your own medical decisions.

Preparing an Advance Directive

Advance directives are written instructions on your health care. These instructions include the name of someone who can make health care decisions for you if you are unable to do so. An explanation of these documents and blank forms can be found here. If you have an advance directive, please provide it to you doctor. If you do not have an advance directive prepared, you can prepare one and have it added to your medical record.

If you have any questions or would like further assistance, you may contact the Care Management Department at 410-368-3157.
